Update Voter Information

Address Change:  (Voter's address changes within the state of Florida)
May be changed by phone, email, written correspondence, voter information card, Florida Voter Registration Application Form (Español) or a vote-by-mail ballot request form from the voter.  With the exception of changes made by phone or email, the voter’s original signature is required.  Requests made by phone, email, absentee request or written correspondence must include the voter’s date of birth or voter registration number.

Name Change:
May be changed by written correspondence, voter information card or Florida Voter Registration Application Form (Español). The voter’s original signature is required. Written correspondence must have the voter’s date of birth or voter registration number.

Party Change: 
May be changed by written correspondence, voter information card or Florida Voter Registration Application Form (Español). The voter’s original signature is required. Written correspondence must have the voter’s date of birth or voter registration number.

Date of Birth Correction: 
May be changed by phone, written correspondence, vote-by-mail ballot request form, voter information card or Florida Voter Registration Application Form (Español). The voter’s original signature is required on all but changes made by phone. Written correspondence must have the voter’s date of birth or voter registration number.

Signature Update: 
It is very important to update your signature on file, as it may change throughout the years. Signatures on your registration record are used to verify signatures on petitions, vote-by-mail ballots and provisional ballots. If your signatures do not match, your petition or ballot will not count. To update your signature, submit a Florida Voter Registration Application Form (Español) to the Department of Elections. The voter's original signature is required.

Replacement Voter Information Card: 
May be requested by written correspondence or Florida Voter Registration Application Form (Español). The voter’s original signature is required.

Vote-By-Mail Ballot Request: 
A vote-by-mail ballot may be requested by the voter or with the voter’s permission, by a member of the voter’s immediate family or the voter’s legal guardian.  It may be requested by phone, email, on the department’s web site or by written request. The request must have the voter’s date of birth and residence address. Requests made by someone other than the voter must include their name, address, relationship to the voter, and if available, their drivers license number.  Written requests must have the voter’s/requestor’s signature. 

  • Ballots mailed to an address other than one on the voter’s record must be submitted by signed written notice.  Persons in the military or residing overseas are exempt from this requirement. 

  • Address changes may be made from a vote-by-mail ballot request if made by the voter.

Voter Registration Cancellation: 
May be cancelled by a written request from the voter. The voter’s original signature is required.
