How to Correct a Signature Discrepancy on your Returned
Vote-By-Mail Ballot
  • If you returned your vote-by-mail ballot to your supervisor of elections, but their is a signature discrepancy on the Voter's Certificate on the return envelope, your ballot will not count unless you complete and return the Affidavit for Signature Cure for a Vote-By-Mail Ballot no later than 5 pm on the day before the election. 

    • Please follow the instructions on the form carefully, as failure to follow these instructions may cause your ballot not to count.

    • Once the affidavit has been completed, return to the Department of Elections via mail, fax, email or in person.
